Ceramides are the major lipids of the stratum corneum, the outermost layer of the epidermis. During keratinization, almost all epidermal phospholipids, which are abundantly found in the basal layer, are lost, while ceramides resynthesized from phospholipid intermediates are retained within the stratum corneum (SC). Ceramide-rich intercellular lipid sheets are believed to be particularly important in maintaining the structural integrity of the epidermal barrier. Depletion of ceramides in stratum corneum (SC) has been implicated as an etiological factor in dryness and barrier disruption in skin disorders like atopic dermatitis (AD).

Supplementation with gromwell root extract ameliorates stratum corneum (SC) hydration while partially increasing ceramides. The study found that SC hydration values were much higher in the gromwell group than in the placebo group, and SC ceramide components were significantly increased in the gromwell group. The findings suggest that increasing the SC ceramide component may help improve skin hydration after 10 weeks of gromwell intake. Results of clinical studies suggest that these deficiencies can be addressed through the use of proper moisturizers that have been shown to ameliorate skin hydration, reduce sensitivity to irritation, and restore SC integrity. This indicates that supplementation with gromwell root extract improved SC hydration, significantly increased SC ceramides, and relieved pruritus symptoms.
